Each disc has a title labelled on it:

*Disc 1 (LC2602) - Whispers Begin
*Disc 2 (LC2603) - Echoes of Discovery
*Disc 3 (LC2604) - Harmony of Revelation

Is there a way to reflect that on the tracklist ?

So far, I've only seen people do that in the notes section.

I would put it in the notes section along the rest of the credits

Actually, isn't this the case with Parasite Eve? I think the various tracks are under the names of the various days.

I think so. Some older soundtracks also have these chapters like from Konami's Suikoden 1 and 2. The original Shin Megami Tensei also had its 2-disc soundtrack labled "Law Disc" and "Chaos Disc". I'm sure there are other release similar to these.
