Uploaded the booklet scans. Not the best quality unfortunately, but the booklet page images themselves are a bit washed out.

I've added in my scans of the disc, obi, back of case slip and inner-tray side of the case slip too.

I can re-scan the booklet if needed or wanted, same thing goes for any of the other Drag-on Dragoon or NieR related CDs.

Our track titles for 12 and 13 were incorrect in both English and Japanese. They're 地上/Ground, not 上空/Sky. Same issue with the two-disc reprint (which I assume was just copied from here), which I've also fixed.
