Can anyone who owns this confirm which tracklist is correct? The differences are mostly cosmetic (quote vs. parentheses) except for a slight wording difference in the first and last tracks.

The manual for the game doesn't include any sort of tracklist for the disc, so I personally assumed that one didn't exist. I've just been going off of the track names from previous releases / OSTs. (Hence listing the first track as Launch Out, instead of Feedback)

Are there tags written directly to the disc? If so, how does one go about reading / accessing them?

They read just fine on Winamp straight from the disc. (Goto "Play -> Audio CD [Drive]")

Says "Ishiwatari Daisuke" (LOL) was the artist for all the tracks.

The inner ring of the CD has "CTDX-004651" engraved, so I changed the catalog number to it.

Just guessing here, but that's probably CDDB information, not CD-Text from the disc.
