I added scans and credits for this one and Fuga OST Vol.1.

One thing that's interesting to note is that despite there being a graphic printed on the tray side of the back insert, the tray itself is opaque, black plastic, so all you can see is a tiny piece of the design through the hole in the middle where the disc sits. Volume 1 is the same way.
